I got this several times playing with XMMS (www.xmms.org). This happens
much often with 1.2.2 and 1.2.1 then with 1.2.0 or 1.0.x. Just listen to
mp3 and play games with Preferences/Plugins etc ... (many ways to crash
it, find your very own one). I reported this to xmms team and NO respond
from them. XMMS was compiled from src.rpm. No other program crashes my
Xserver, just this one ...

xmms-1.2.x (1.2.[12]) (no setuid, no RT, no DGA)
